When you refer someone, you are trusting another provider to understand the person properly, not just fill a shift.
At Rahma Care, we look at the support required, the participant’s preferences, the worker fit and whether we are genuinely the right provider before anything is confirmed.
Because a good start is not about moving quickly. It is about getting the fit right from the beginning.

Who We May Be a Good Fit For
We consider each referral individually, taking into account the person’s support needs, preferences, circumstances and the kind of worker who is likely to be the right fit.
Psychosocial Disability
​Supporting people with psychosocial disability is a core part of Rahma Care. We understand that the right support may look different from day to day, so we place strong emphasis on consistency, trust and finding a worker the person feels comfortable with. The aim is to help people stay connected with everyday life, their community and what matters to them.
Different Life Circumstances
​We actively seek referrals for people whose circumstances may include current or past substance use or involvement with the justice system. Rather than making assumptions from someone’s history, we look at what is happening now, what support is being requested and what approach will give the worker-participant relationship the best chance of working well.
Diverse Communities
Culture, language, family context and personal preferences can all shape what good support looks like. These are considered when discussing the referral and when thinking about worker compatibility.
Languages spoken across our team include English, Urdu, Pashto, Punjabi, Hindi and Mandarin.
Suitability is considered individually. If a person’s support needs sit outside Rahma Care’s role, worker capability or current capacity, we will be clear about that before support is agreed.
WHAT REFERRERS CAN EXPECT
What You Can Expect After a Referral
A referral does not end once support starts. The worker still needs to be the right fit, important changes need to be followed up, and the right people need to know what matters. That is how we approach the referrals we accept.
A Clear Suitability Decision
We consider the participant’s support needs, preferences, location, requested times and what will be expected of the worker before confirming support. Capacity alone does not determine suitability.
Matching Beyond Availability
​Before support begins, we look carefully at who is likely to be the right fit for the participant, taking into account experience, personality, communication style, culture and personal preferences. Where possible, we keep that worker relationship consistent, and if the match is not working well, we review it rather than expecting the participant to simply adjust.
​Communication That Keeps You Informed
​With the participant’s consent, referrers can receive regular updates, relevant progress notes or support summaries, and early communication when something changes. Where an issue needs attention, we explain what has been followed up and what happens next.
Follow Through When Something Changes
When something about the support is not working well, we address it early and work with the participant and the people they choose to involve to understand what needs to change. We then follow it through and keep everyone clear about what is happening next.

We review the participant’s support needs, preferences, requested times and current capacity to determine whether the referral may be suitable.
We Clarify What Matters
Where more information is needed, we speak with the participant, authorised contact or referrer as appropriate so expectations are clear before support is agreed.
We Confirm the Next Step
We clearly advise whether Rahma Care can proceed. Where support is suitable, we confirm the next arrangements, including worker matching and proposed commencement.
We confirm support once we have a clear understanding of what is needed and know we have the right capacity to provide it well.
WORKING TOGETHER
Working With the People the Participant Chooses
Good Core Support often sits alongside the work of Support Coordinators, families, allied health professionals and other services. With the participant’s consent, Rahma Care can work with the people they choose to involve so relevant information, agreed everyday strategies and important changes are understood.
Where an allied health professional has recommended an everyday strategy relevant to the participant’s support, workers can assist with agreed strategies that sit within their role, skills and training.
A conversation can help determine whether Rahma Care is suitable before a referral is submitted.
